All untoward events must be recorded in detail on the anaesthetic chart (and continued in the medical notes if needed). These untoward events serve as a warning to future anaesthetists and as a legal record of exactly what happened during the procedure.

Sufficient detail should be included to enable fellow healthcare workers to establish the sequence of events.
