Answer: Table 1 shows the initial management of choking. Choking is characterized by a history of sudden onset of coughing with respiratory distress. There is usually a history of playing with small objects, although the choking episode may be unwitnessed.

Fig 1 shows the UK Resuscitation Council's Paediatric Choking Treatment Algorithm.

Choking child - assess the effectiveness of coughing:

Ineffective cough
  • Unable to vocalise
  • Quiet or silent cough
  • Unable to breathe
  • Cyanosis
  • Decreased conscious level
Effective cough
  • Crying or verbal response to questions
  • Loud cough
  • Able to take a breath before coughing
  • Fully alert
