Question: How do you assess burns in children?

Answer: Use the Rule of 9's diagram (Fig 1) to estimate % body surface area (BSA) burned. Assess the depth and classify the type of burn present (all types may be present):

  • Superficial first degree burns (mild pain, minimal swelling)
  • Partial thickness burns (superficial painful scalding injuries with blisters; or less painful deep burns that may require skin grafting)
  • Full thickness burns (dry, leathery and painless; will need skin graft to heal)
  • Take note of burns involving the airway (early intubation before swelling severe), or circumferential burns (surgical release may be required)

Cover the burn loosely with non-adherent dressing (clingfilm, banana leaf, sterile towels) to keep clean and provide some pain relief.

Beware heat loss form large burns.
