You could use the scoring system shown in Fig 1:

• Each vital sign parameter receives a score of 0,1 or 2
• For example a heart rate of 35, gets score of 2
• A Systolic BP 155, gets a score of 1
• Add individual scores to give a total
• A total score of >2 needs review
