Question: What important issue do you need to be aware of when using ketamine as an additive?

Answer: The ketamine added to caudal/spinal anaesthesia has to be preservative-free.

The preservative that is present in regular ketamine can cause serious damage to the spinal cord.

Any drug used for spinal, epidural or caudal anaesthesia must be free of preservatives. It should also be clearly indicated on the label that it is safe to use via that route.
